Nursing documentation is vitally important, and it is essential that all aged-care facilities have a clear vision of their objectives and requirements with respect to this aspect of care. Many time-consuming and costly ‘documentation issues’ can be avoided if a clear vision and accompanying guidelines are provided for all nursing staff. To avoid incomplete, inconsistent, ambiguous, and reactive documentation, it is essential that nurses understand the documentation requirements of the organisation in which they work.
